About the Role The Care Management Associate is a critical component of our Outreach Hub, responsible for providing comprehensive care coordination through telephonic outreach and enrollment of eligible members. As a healthcare professional with a passion for delivering exceptional customer service, you'll be the face of our organization, connecting with members, facilitating case assignments, and ensuring seamless transitions to care managers. Your expertise will help drive the successful implementation of care plans, promoting effective utilization of healthcare services and supporting quality effectiveness. Key Responsibilities Conduct telephonic outreach to Medicaid members, engaging them in care management programs and facilitating enrollment Provide exceptional customer service, addressing member inquiries and concerns in a timely and empathetic manner Facilitate case assignments and warm transfers to care managers, ensuring seamless transitions and continuity of care Maintain accurate and complete documentation of member interactions, adhering to regulatory and accreditation requirements Support team members by answering telephone calls, researching information, and assisting in problem-solving Demonstrate a strong understanding of care management processes, including privacy and confidentiality, quality management, and regulatory compliance Essential Qualifications To succeed in this role, you'll need: 2 years of experience in the healthcare field, preferably in a medical office, hospital setting, or medical billing/coding environment Proficiency in Microsoft Office applications, including Word, Outlook, and Excel, with experience in data entry and documentation within member records Familiarity with basic medical terminology and concepts used in care management Flexibility to work occasional nights and weekends outside of standard business hours (8:00 am to 8:00 pm) Strong organizational skills, with effective verbal and written communication skills A high school diploma or equivalent GED Preferred Qualifications To take your application to the next level, consider: Bilingual skills (Spanish), with the ability to pass a written and oral exam Excellent communication, telephonic, and organizational skills, with the ability to manage multiple priorities and adapt to change with enthusiasm A proven track record of meeting daily metrics with speed, accuracy, and a positive attitude Strong customer service skills, with a focus on coordinating service delivery, addressing customer issues, and promoting positive outcomes for members The ability to work independently and competently, meeting deliverables and deadlines while demonstrating an outgoing, enthusiastic, and caring presence telephonically Effective participation in a multi-disciplinary team, including internal and external participants What We Offer At CVS Health, we're committed to supporting the well-being and success of our colleagues.
