To learn more, visit Role Scope: Coordinates Quality Improvement activities to ensure quality of care standards are consistently achieved with maximum efficiency. Oversees the Quality Improvement Program to ensure compliance with PACE and other applicable regulatory standards. • Oversees the annual evaluation and ongoing revision of the Quality Improvement (QI) Plan to ensure achievement of organizational goals and alignment with current standards • Leads the Quality Improvement Committee, providing administrative oversight and strategic direction • Keeps current on state and federal QI regulations, communicating changes to staff and updating internal processes as needed • Ensures compliance with mandated QI reporting requirements at both state and federal levels • Monitors and drives improvements in participant outcomes, using organizational metrics to guide performance and identify opportunities for growth • Analyzes QI data to identify trends, measures impact, and implement effective interventions aimed at improving clinical outcomes • Manges participant safety events, including coordination of quarterly reporting and follow-up analysis. • Partners with Center Leadership and Population Health teams to assess clinical data, track progress toward performance thresholds, and implement corrective action when necessary. • Collaborates across departments to launch and sustain quality initiatives that enhance service delivery and operational excellence. • Other duties as assigned Qualifications: • Licensed Vocational Nurse (LVN), Registered Nurse (RN) preferred • 1+ years of relevant experience, such as in quality, healthcare, or PACE • PACE program experience preferred • Willingness to work in a hybrid work environment • Proof of medical clearance for communicable diseases, including a TB test. • Proof all immunizations are up to date Nice to have: • Strong understanding of PACE regulations and DHCS requirements • Advanced analytical and problem-solving skills • Excellent written and verbal communication skills • Previous experience in PACE or similar program serving older adults with complex needs Compensation: We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $93,000 -$110,000 and is bonus eligible. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
