Director, Clinical Operations -Care At Home Solutions

Director Care At Home Services leads the development and execution of a Health delivery system within COPE Health Solutions and CHS Medical Group, focusing on improving the health outcomes of a specific group of individuals. This role involves leveraging data analytics, understanding the healthcare landscape, and collaborating with various stakeholders to implement targeted interventions and programs. FLSA Status Exempt Salary Range $140,000 - $180,000 Reports To Principal and Executive Vice President Direct Reports N/A Location California/New York Travel Up to 80% Work Type Regular Schedule Full Time Key Responsibilities: • Develop and implement a Care delivery strategy that aligns with the organization's goals and objectives. • Use data analytics to identify trends, patterns, and opportunities for improvement within the population. • Design and implement targeted interventions and programs to address specific health needs and improve outcomes. • Collaborate with various stakeholders, including physicians, nurses, care managers, and payers, to ensure effective implementation of the strategy. • Monitor the performance of the population health strategy and makes adjustments as needed. • Work to improve the quality and cost of care for the population. • Engage with external stakeholders, such as community organizations and regulatory agencies, to promote the care delivery strategy and health outcomes. • Mentor and develop talent within the organization to support the population health strategy. • Understands and manages the financial implications of the population health strategy. • Ensures compliance with relevant regulations and policies. • Develop clinical and operational processes for the CHS Medical Group and the Care at Home Solutions • Dyad with CMO and EVP of CHS Medical in developing, implementing, and maintaining standards and training for the CHS Medical Group and Care at Home Solutions division, including provision of technical quality assurance requirements. Required Skills & Qualifications : • Leadership: Strong leadership skills and the ability to motivate and engage a team. • Communication: Excellent communication and interpersonal skills to effectively collaborate with diverse stakeholders. • Analytical: Strong analytical skills to interpret data and identify trends. • Strategic Thinking: Ability to develop and implement strategic plans. • Project Management: Strong project management skills to oversee the implementation of programs and initiatives. • Knowledge of Healthcare: In-depth knowledge of the healthcare industry, including managed care, value-based care, Medicare and Medicaid in California Qualifications • 7-11+ years of work health care administration experience, in addition the ideal candidate will have at least 4+ years of health care consulting experience • Clinical degree (e.g., PA, NP, RN, LCSW, PhD) • Master’s degree in an applicable field (MBA, MSN, MPH, MHA or similar) strongly preferred. • Ability to travel up to 80%. • Flexibility to work evenings and weekends as necessary. • Bilingual- Spanish speaking is a requirement Benefits: As a firm passionate about health care, we’re deeply committed to the health and wellness of our own team members.
