Discharge Planner, LVN - After hours
About the position The Inpatient Discharge Planner, LVN - Afterhours plays a critical role in ensuring a seamless transition for patients from acute Hospital care to their next phase of recovery, whether that be home, a rehabilitation facility, or another care setting. This position involves assessing patients need , coordinating post-discharge services, and collaborating with healthcare providers, patients, and families to develop and implement comprehensive discharge plans. Responsibilities • Reviews Nurse Case Manager’s documentation to assist with determination of discharge needs • Implement individualized discharge plans in collaboration with Case Management, physicians, nurses, hospital case management teams, and other healthcare professionals. • Obtain orders and ensure arrangement of post-discharge services such as home health, medical equipment, or placement in skill nursing or other lower level of care based on patient needs. • Pull records and obtain any necessary forms required for discharge planning. • Follow up on any discharge needs to ensure completion. • Provide clear, compassionate guidance to patients and their families about discharge plans and follow-up care instructions. • Participate in daily rounds to ensure timely discharge planning. • Maintain accurate and timely documentation of discharge plans, interventions, and communications in patient’s case. • Identify and address barriers to discharge, escalating to Case Management or Medical Directors for resolution as needed. • Communicate post discharge arrangements to hospital case management, nursing staff, or physicians to ensure a smooth transition. • Performs all other duties as directed by management. Requirements • Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program. • Current California Licenses • Acute Care Required • Hospital experience • Care planning • Discharge planning • Typing 40 words per minutes with accuracy. • Knowledge of computers, faxes, printers and all other equipment. • Knowledgeable in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point). • Ability to deal with responsibility with confidential matters. Nice-to-haves • Knowledge of Healthcare and Managed Care preferred.
