About the Role We are seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Home Health Care Coordinator (RN) Remote. As a Home Health Care Coordinator, you will be responsible for coordinating care for members with complex medical conditions, substance abuse, and behavioral health needs. Your expertise and dedication will play a vital role in ensuring that our members receive the highest quality care, while promoting their overall well-being and quality of life. Job Summary The Home Health Care Coordinator (RN) Remote will be responsible for supporting our Washington State Plan, working closely with multidisciplinary teams to assess, facilitate, plan, and coordinate care for members with high need potential. This role requires a strong understanding of behavioral health services, substance abuse, physical health/disease management, and long-term care. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with members, caregivers, physicians, and other healthcare professionals. Key Responsibilities Completes comprehensive assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member's health or psychosocial wellness, and triggers identified in the assessment. Develops and implements a case management plan in collaboration with the member, caregiver, physician, and/or other appropriate healthcare professionals and member's support network to address the member needs and goals. Conducts face-to-face or home visits as required. Performs ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. Maintains ongoing member case load for regular outreach and management. Promotes integration of services for members including behavioral health care and long-term services and supports/home and community to enhance the continuity of care for Molina members. Facilitates interdisciplinary care team meetings and informal ICT collaboration. Uses motivational interviewing and Molina clinical guideposts to educate, support, and motivate change during member contacts. Assesses for barriers to care, provides care coordination and assistance to member to address concerns. 25-40% local travel required. RNs provide consultation, recommendations, and education as appropriate to non-RN case managers. RNs are assigned cases with members who have complex medical conditions and medication regimens. RNs conduct medication reconciliation when needed. Essential Qualifications To be successful in this role, you will need to possess the following essential qualifications: Graduate from an Accredited School of Nursing. Active, unrestricted State Registered Nursing (RN) license in good standing. Valid driver's license with good driving record and ability to drive within applicable state or locality with reliable transportation. 1-3 years of experience in case management, disease management, managed care, or medical or behavioral health settings. Preferred Qualifications We are looking for candidates who possess the following preferred qualifications: Bachelor's Degree in Nursing. 3-5 years of experience in case management, disease management, managed care, or medical or behavioral health settings. Active, unrestricted Certified Case Manager (CCM) certification. Skills and Competencies To excel in this role, you will need to possess the following skills and competencies: Excellent communication and interpersonal skills. Strong assessment and problem-solving skills. Ability to work effectively in a fast-paced environment. Strong knowledge of behavioral health services, substance abuse, physical health/disease management, and long-term care. Ability to work collaboratively with multidisciplinary teams. Strong organizational and time management skills. Ability to maintain confidentiality and handle sensitive information.
