Our staff, comprised of Long Islanders with an unmistakable mix of courage and heart, care about the people right here in our community and are dedicated to providing personalized comfort. **Job Summary** We are seeking an experienced and skilled Registered Nurse to join our team in the Step Down Unit, working full-time day shifts. As a Registered Nurse in this role, you will be responsible for executing professional nursing care in accordance with physician treatment plans to provide for the comfort and well-being of patients and their family or support system. You will assess patients' needs, plan and implement treatment plans, observe and evaluate patients' progress, and document pertinent observations. You will work collaboratively with the interdisciplinary health team to meet the holistic needs of patients and direct the delivery of nursing care by other members of the nursing team. **Key Responsibilities** * Collect patient health data by involving the patient, family, and other healthcare providers * Perform initial assessments based on patients' presentations and record relevant data on the nursing component of the Interdisciplinary Admission Assessment * Demonstrate awareness of the special needs of each age group or culture served, as well as those patients with disabilities * Perform ongoing assessments as established by department policy and as warranted by patients' responses * Incorporate discharge planning into the admission process * Analyze assessment data to determine problem identification and identify expected outcomes individualized to the patient * Develop a plan of care that prescribes interventions to attain expected outcomes and collaborate with patients, families, and other healthcare providers in developing the plan of care * Formulate a family-centered plan of care consistent with the assessment of the patient's needs and the physician's treatment plan * Incorporate teaching and learning principles in the plan of care based on the identified learning needs and developmental level * Assist patients and families in identifying and securing services to address health-related needs * Communicate the plan of care to patients, families, and other healthcare providers to promote continuity of care * Document the initial plan of care and provide updates as required * Establish realistic goals, set appropriate priorities, and use time efficiently * Implement the plan of care for each patient and perform appropriate patient monitoring * Adapt nursing procedures to meet the needs of individual patients * Provide education to patients and families utilizing the educational assessment * Recognize hazards to patient safety and take action to maintain a safe environment * Work collaboratively with other team members to implement interventions * Assign or delegate tasks as defined by the state nurse practice act and according to the knowledge and skills of the caregiver * Demonstrate knowledge regarding drug administration, incompatibilities, contraindications, and adverse effects * Seek out appropriate resources when uncertain regarding an aspect of medication administration * Comply with medication policy and procedure for order transcription/verification, drug administration, and documentation * Evaluate patients' progress toward attainment of outcomes and document patients' responses to interventions and revisions to the plan of care * Identify barriers to achieving desired patient/family outcomes and utilize resources to diminish the negative impact on the patient/family * Communicate with and include patients, families, and other healthcare providers in the evaluation process * Provide care that demonstrates ethical beliefs and respect for patients' rights * Demonstrate and participate in activities that support customer satisfaction * Participate in the evaluation of the quality and effectiveness of nursing practices and support measures to improve practice * Demonstrate the ability to maintain effective interactions with staff on the unit and in other departments * Interact with and contribute to the professional development of peers and other healthcare providers * Demonstrate the ability to act as a facilitator when dealing with other staff members regarding issues concerning patient care and/or functioning on the unit **Essential Qualifications** * Professional knowledge of nursing theory and practice acquired through completion of a two-year training at an accredited School of Nursing * Current New York State license or permit as a Registered Professional Nurse * Interpersonal skills necessary to effectively interact with patients and their families that demonstrate recognition of cultural diversity * Ability to clearly communicate in verbal and written format * Analytical ability to assess and develop solutions to clinical situations or unit problems * Maintains knowledge and proficiency in the clinical area and age group served * BLS certification and de-escalation training * ACLS (ICU, Float pool, etc.) * Ability to frequently lift/carry more than 75 lbs. * Ability to frequently push/pull more than 100 lbs. * Ability to frequently bend, squat, kneel, climb steps/ladder, and reach above shoulders **Preferred Qualifications** * Experience working in a Step Down Unit or similar setting * Strong communication and interpersonal skills * Ability to work effectively in a fast-paced environment *
