Key Responsibilities As an LPN in our home health care team, your role will be multifaceted, focusing on delivering high-quality patient care and supporting our interdisciplinary team. Your key responsibilities will include: Verifying the plan of care with the Registered Nurse (RN) prior to each visit to ensure alignment with patient needs. Collaborating frequently with physicians and RNs to ensure that patient needs are met, and notifying them promptly of any changes in patient condition. Teaching patients and caregivers self-care techniques to promote independence and manage their health effectively. Checking vital signs and monitoring patient health status. Reviewing all medications and performing medication reconciliation at each visit to ensure safe and effective medication management. Performing specialized procedures as ordered by physicians, such as foley catheter changes, using aseptic technique. Preparing equipment and materials for treatments, maintaining a sterile environment. Completing documentation in a timely and accurate manner to ensure continuity of care. Qualifications and Requirements To be considered for this rewarding role, you must meet the following essential qualifications: One year of clinical LPN experience, demonstrating your ability to provide high-quality patient care. Current LPN license in the state(s) where you will be working, ensuring compliance with regulatory requirements. Current CPR certification, reflecting your commitment to maintaining life-saving skills. Valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient visits.
