Our team is passionate about making a difference in the lives of our patients and their families, and we are seeking like-minded individuals to join our team. **Job Summary** As a Patient Care Coordinator, you will be responsible for providing individualized support to new and existing patients, producers, and centers, comparable to their remedies, clinical services, and program requests. This role requires a high level of attention to detail, excellent communication skills, and the ability to work independently with minimal supervision. **Key Responsibilities** * Provides individualized support to new and existing patients, producers, and centers, comparable to their remedies, clinical services, and program requests. * Handles references from initial enrollment, through verification and shipment, in accordance with regulatory requirements and understands producer-established qualification measures, including but not limited to, determining family income and applicable% of Government Poverty Level (FPL) rules. * Serves as the single point of contact for producers and centers connecting with any patient, medication, program, or information question, while ensuring strategies and systems are followed; facilitates patient care by planning references, imparting shipment data, evaluating supply needs, checking patient data, and triaging patients to a pharmacist, as necessary. * Makes and cycles through fully appointed reference and request stages, including re-orders received by electronic information feed, fax, mail, and phone; collaborates intimately with producer/center point Caseworkers to ensure smooth communication and minimize service interruptions. * Makes and distributes daily Scaffold Report and Quick Start reports for review of patient qualification for Division Extension and Quick Start projects, as well as handling through all reference and request stages all Extension and Quick Start references and re-order references. * Processes NMDP references from admission through request stages, including sending delivery passes to NMDP to ensure compliance with program requirements; participates on standard rhythm calls with the producer to discuss references in process, issues, and questions connected with the program. * Completes monthly benefit review to determine patient's qualification for patient assistance programs (PAP), including review of patient financial records. * Communicates with pharmacists to inform them of any changes in a patient's condition, compliance issues, or side effects and moves patients directly to a pharmacist for guidance, as necessary. * Assists with patient activities, including follow-up for patient's next order/pick-up, initiate the release of patient from services, contact a patient's primary care physician's office to arrange delivery of a request, request a letter be sent to the patient if unable to contact and any other updates necessary connected with the patient's care. **Essential Capabilities** * High school diploma from a certified school or equivalent GED * Experience providing exceptional customer care * Excellent written and verbal communication skills * Ability to focus and manage multiple tasks * Experience creating ways to achieve objectives with minimal oversight, relying on oneself to complete goals and determining when escalating issues is necessary **Preferred Capabilities** * Experience in working out benefit utilization adjustments, co-pays, lengths of supply, and other healthcare numerical calculations * Possession of a current pharmacy professional license or certification as required by state Board of Pharmacy * At least 1 year of experience in patient care, healthcare, retail, or customer service-oriented job * Strong empathy and compassion **What We Offer** * Competitive salary of $35/hour * Opportunities for career growth and professional development * Collaborative and dynamic work environment * Comprehensive benefits package, including medical, dental, and vision insurance * Generous paid time off and holidays * Recognition and rewards for outstanding performance **How to Apply** If this role sounds like a perfect fit, don't hesitate.
