Job Overview As a Physical Therapist in Home Health Care, you will be responsible for providing direct patient care utilizing age-specific standards of practice for physical therapists. You will work independently and collaboratively with other team members to obtain patient-centered goals within Henry Ford Allegiance Health. This is a full-time, day shift position located in Jackson, MI, with the flexibility to work remotely. Key Responsibilities: Provide direct patient care utilizing appropriate age-specific standards of practice for physical therapists in regard to all patient populations served Work independently and collaboratively with other team members to obtain patient-centered goals within Henry Ford Allegiance Health Conduct patient assessments, develop treatment plans, and implement interventions to achieve optimal patient outcomes Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care Participate in ongoing professional development to stay current with best practices and advancements in physical therapy Essential Qualifications: To be successful in this role, you will need: A Master's or Doctorate degree from an accredited Physical Therapy program Minimum of one year of experience providing physical therapy in a healthcare setting Current Michigan licensure for Physical Therapy Valid Michigan driver's license and dependable vehicle insured as required by the State of Michigan Ability to problem-solve and effective verbal and written communication skills Knowledge about the care of neonate, pediatric, adolescent, adult, and geriatric patients Preferred Qualifications: While not required, the following qualifications are highly preferred: Minimum of two years of experience as a Physical Therapist Experience in homecare Skills and Competencies: To excel in this role, you will need to possess: Strong clinical skills and knowledge of physical therapy principles and practices Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong problem-solving and critical thinking skills Ability to adapt to changing patient needs and priorities Career Growth Opportunities and Learning Benefits: At Henry Ford Health System, we are committed to supporting the growth and development of our employees.
