Key Responsibilities: Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans that promote optimal health outcomes. Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination and communication. Provide education and support to patients and their families, empowering them to take an active role in their care and make informed decisions about their health. Develop and implement strategies to improve patient outcomes, reduce hospital readmissions, and enhance the overall quality of care. Participate in ongoing quality improvement initiatives, staying up-to-date with the latest evidence-based practices and industry trends. Qualifications and Requirements To be successful in this role, you will need: A graduate degree from an approved school of professional nursing and current licensure to practice as a registered nurse in the state of Texas. At least one year of experience as a Registered Nurse, with a strong preference for experience in home health care or case management. Required certifications and licensures, including valid CPR certification and a licensed driver with an insured vehicle. Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced, dynamic environment. A strong commitment to delivering patient-centered care and promoting health equity, with a passion for working with diverse populations and addressing the social determinants of health.
