Job Overview We are seeking an experienced and compassionate Telephonic Nurse Practitioner to join our team. As a Nurse Practitioner, you will play a vital role in delivering high-quality, patient-centered care to our members during evening, overnight, weekend, and holiday hours. Key Responsibilities Provide telephonic care and direction to members and facility staff during designated hours, ensuring timely and effective support. Utilize electronic medical records (EMR) proficiently to deliver acute care and document patient interactions accurately and comprehensively. Conduct comprehensive assessments, manage medical and behavioral conditions, and develop care plans that meet the unique needs of each patient. Collaborate with physicians, other healthcare professionals, and family members to ensure coordinated care and optimal patient outcomes. Participate in quality initiatives and contribute to the development of best practices in telephonic care. Maintain current knowledge of clinical practices, guidelines, and protocols, applying this expertise to improve patient care and outcomes. Scheduling and Work Environment This position requires flexibility and adaptability, with scheduling that includes weeknights, weekends, and holidays. You will be expected to work a mix of shifts, including overnight coverage, to ensure our members receive continuous support. Essential Qualifications Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program. Active and unrestricted New York license as a Nurse Practitioner, with the ability to obtain licenses in other states as required. Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with specialization in Family, Adult, Gerontology, or Adult-Gerontology Acute Care. Current DEA licensure or the ability to obtain it post-hire, as per state regulations. At least 2 years of clinical experience as an Advanced Practice Clinician (APC), with a proven ability to work collaboratively and independently. Proficiency in electronic medical records and excellent computer skills. Ability to work scheduled shifts, including nights, weekends, and holidays, with a commitment to flexibility and adaptability. Preferred Qualifications 3+ years of clinical experience as an APC, preferably in settings such as SNF, ICU, LTACH, Hospitalist, Emergency, ACNP, or Critical Care. Recent experience in meeting the complex medical, behavioral, and social needs of patients. Experience working with seniors in acute care facilities or nursing homes. Understanding of advanced illness, end-of-life discussions, and palliative care principles. Proven ability to develop and maintain positive relationships with patients, families, and healthcare teams. Adaptability to change and a strong commitment to continuous learning and professional development. Compensation, Perks, and Benefits We offer a competitive salary range for this position, from $88,000 to $173,200 annually, depending on experience and qualifications.
