We're seeking an experienced Virtual Patient Manager to join our team and play a critical role in managing and growing our post-acute care performance networks. About the Role: Virtual Patient Manager As a Virtual Patient Manager, you will be responsible for overseeing the day-to-day management of several post-acute markets, fostering strong relationships with Skilled Nursing Facilities (SNFs) and Home Health Agencies (HHAs), and driving performance improvements within our networks. This remote opportunity allows you to work from the comfort of your own home, with less than 10% travel required. Key Responsibilities: Manage and develop relationships with SNFs and HHAs to drive network growth and performance. Collaborate with internal client teams to ensure seamless communication and coordination. Engage with SNF Interdisciplinary Teams to optimize length of stay for ACO and Value-Based Care patients and ensure patient stability post-discharge. Lead monthly meetings with performance network facilities/agencies to report outcomes and recommend process improvements. Identify high-priority care redesign interventions to enhance performance and efficiency within local networks. Utilize CVS Health's patient tracking software platform daily to inform decision-making. Collaborate with Clinical Strategists and Site of Care Managers to communicate PAC interventions to partners. Confirm start of care dates for HHAs, including documentation of any delays or readmissions. Review and initiate SNF escalations for patients with Social Determinants of Health (SDOH) or clinical barriers to discharge. Analyze readmission reasons and implement process improvements with SNFs/HHAs. Essential Qualifications: 6+ years of experience in healthcare and/or business, with a strong background in patient assessment, evaluation, and case management. Strong critical thinking skills to analyze medical data and reports. Ability to exercise independent judgment and make informed decisions. Bachelor's degree required. Residency in South Carolina is mandatory. Preferred Qualifications: Understanding of Care Transition Models (e.g., Naylor, Coleman, Boost). Case management and discharge planning experience.
