About the Role We are seeking an experienced VNA Registered Nurse to join our Hospice Care team. As a Hospice Care Specialist, you will play a vital role in providing comprehensive nursing care to our patients and their families in their place of residence. This is a remote position that offers an immediate start for the right candidate. Key Responsibilities Provide comprehensive nursing care to hospice patients and their families in their place of residence, including developing initial plans of care and supervising hospice aides. Collaborate with the interdisciplinary team and hospice physician to manage end-of-life symptoms and provide holistic support to patients and their families. Conduct holistic assessments of patients and their families, considering psychosocial, environmental, spiritual, and financial needs. Review and update plans of care with the interdisciplinary team and hospice physician to ensure that patients and their families receive the best possible care. Communicate and educate patients and their families on their plan of care, providing guidance and support as needed. Supervise and coordinate the care of hospice aides, ensuring that patients receive the highest level of care. Qualifications To be successful in this role, you will need to possess the following qualifications: A graduate of an approved school of professional nursing, licensed as a Registered Nurse by the State in which practicing, with a Baccalaureate degree preferred. Two years of experience as a professional nurse in nursing practice or previous community health experience as a professional nurse, preferred.
