Job Summary As a Registered Nurse with our client, your primary responsibilities encompass conducting evaluations to determine nursing needs, developing and revising individualized Plans of Care, performing specialized nursing procedures, and managing on-call duties. You will be engaged in direct patient care and coordination of multidisciplinary care, ensuring compliance with agency policies and regulatory standards. Key Responsibilities • Conduct initial evaluations and regular re-evaluations to determine nursing needs. • Develop, initiate, and revise individualized Plans of Care. • Perform specialized nursing procedures, including preventive and rehabilitative care. • Provide direct patient care and manage on-call duties (nights, weekends, holidays) as assigned. • Maintain timely clinical notes, progress reports, and care conference summaries per agency policy. • Accurately complete OASIS assessments and use ICD-10 coding within defined timeframes. • Process physician orders, certifications/recertifications, and discharge summaries. • Adhere to all established agency policies and regulatory standards. • Act as a liaison between the patient, family, and physician regarding changes in condition. • Coordinate multidisciplinary care by referring patients to PT, SLP, OT, or Medical Social Work as needed. • Lead and participate in patient care conferences and utilization reviews. • Supervise, mentor, and teach junior nursing personnel. • Participate in peer reviews, performance improvement initiatives, and in-service training programs. • Counsel patients and families on managing nursing and health-related needs. Qualifications • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. Experience • Two (2) years nursing experience, preferred. Skills • Technical proficiency in conducting OASIS assessments and ICD-10 coding. • Excellent communication and teamwork abilities. Additional Requirements • 40 hours a week, with flexibility to build your own schedule. • During training, you would be required to drive to Cheyenne 3 days out of the first month. • ON SITE position, must be able to drive to patients homes.
