Healthcare Advocate (Remote 1099)
Job Summary We are seeking a dedicated and experienced Patient Advocate/ Care Advocate (aka case manager) with Spanish capabilities to do patient navigation and patient advocacy for our patients and help improve operations within our telehealth company. We're heavily focused on Medicare patients. Responsibilities and Duties: • Do care navigation for patients. This isn't a managerial role at the moment. It will be after. Lead and manage nursing staff to ensure high-quality patient care and adherence to best practices. • Utilize the Pairtu platform for care coordination, case management, and resource access among individuals and communities. • Conduct outreach through various channels, including community events, home visits, and digital communication. • Provide culturally appropriate health education and resources. • Advocate for individuals and communities, focusing on housing, food security, and other critical needs. • Deliver coaching and support for nutrition, mental health, substance use recovery, and chronic disease management. • Foster community participation in health assessments and education initiatives. • Embrace and practice cultural humility and awareness. Critical Skills and Aptitudes: • Strong reading comprehension and ability to assimilate new information effectively. • Excellent multitasking and organizational skills. • Proficient in both independent and collaborative work settings. • Exceptional communication skills, with a focus on building trust and rapport with diverse clients. • Creative problem-solving abilities and resilience in stressful situations. • Commitment to providing outstanding customer service and maintaining professional boundaries. Qualifications • 5+ years of direct patient management • You have had a case load of more than 20 patients at a time and managed • A background in healthcare, social work, case management, or patient advocacy, with experience in care coordination, community health, or a similar role. • Strong understanding of social determinants of health and experience supporting diverse patient populations. • A deep sense of empathy and a commitment to advocating for individuals who may struggle to advocate for themselves. • Exceptional clinical skills combined with strong organizational abilities, capable of managing multiple priorities while maintaining clear communication and accurate documentation. • Strong knowledge of hospital operations, behavioral health practices, and inpatient care protocols. • Proficient understanding of physiology and diagnostic evaluation processes. • Excellent leadership, communication, and interpersonal skills. • Ability to work collaboratively within a multidisciplinary team environment.
