Provide high-quality and compassionate support to individuals and their families through the High Fidelity Wraparound program. Work closely with individuals to develop and implement personalized care plans that address their specific needs and goals. Serve as a liaison between individuals and their families and the complex healthcare system, helping them navigate through various resources and services. Utilize strong communication skills to effectively collaborate with clients, their families, and other healthcare professionals. Conduct thorough assessments to identify strengths, needs, and barriers to accessing healthcare services. Develop and maintain relationships with community resources to ensure individuals have access to necessary support and services. Facilitate meetings and coordinate care with other providers, including healthcare professionals, social workers, and community organizations. Regularly monitor and evaluate the progress of individuals in the program, making adjustments to care plans as needed. Maintain accurate and up-to-date documentation of all interactions and services provided. Adhere to all relevant laws, regulations, and ethical standards in the provision of services. Attend regular trainings and professional development opportunities to stay current on best practices and strategies for supporting individuals and families.
