Job title: Nursing Assessment Coordinator MDS Company: The Highlands Guest Care Center Job description: BASIC FUNCTION: The Nursing Assessment Coordinator (NAC) is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM). These duties are to be performed in a timely manner consistent with Centers for Medicare & Medicaid (CMS) guidelines and Integrated Health Services (IHS) Standards. This individual also establishes and maintains positive relationships with all members of the health care team. CHARACTERISTIC DUTIES AND RESPONSIBILITIES ESSENTIAL FUNCTIONS * Maintains current MDS status of each patient/resident according to state and federal guidelines as required under the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable. • Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessment. • Communicates schedule to all other departments and families, as required. • Implements system to monitor for changes in patient/resident condition. • Responds to patient/resident change in condition by coordinating patient/resident reassessment and re-evaluating plan of care. • Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered that most accurately reflect the status of the patient/resident. • Accurately completes portions of MDS and RAPs as assigned per IHS Standards. • Assures compliance with federal, state, Medicare, Medicaid regulations and IHS standards regarding completion, accuracy, electronic submission, and timeliness while coordinating the RAI process. • Coordinates and develops plan of care as required. • Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is in file. • 11.Maintains proficiency in software programs. • Communicates information to nursing administration, business office, and facility administration regarding assessments, care plans, updates and revisions. • Assures competency of all staff involved in the RAI process and educates the interdisciplinary team. • Coordinates and attends daily PPS ITM Meetings. • Attends the (IBRA required ITM. • Maintains assessments on the active clinical record for a period of not less than 15 months. • Electronically transmits MDS data to the state per regulation and IHS Standard • Remains current with industry changes. • Educates a back-up person for the NAC position. MINIMUM SKILLS REQUIRED * Must possess excellent verbal and written communication skills. • Must possess the ability to work independently. • Must possess the ability to lead team of interdisciplinary professionals. • Must possess the ability to coordinate and manage entire RAI process. • Must have excellent organizational and time management skills. • Must have the ability to assess and process information in a logical manner. • Must have flexibility to adapt to frequent change. • Must possess the ability to deal with conflicting events outside realm of control. • Must possess the ability to set goals, monitor progress and obtain established goals within established time frames. • Must possess general clerical skills including typing, telephone and basic computer skills. • Must have excellent attention to detail. • Must possess a positive attitude. MINIMUM QUALIFICATIONS * Current nursing license required; BSN preferred. • Five or more years direct hands-on nursing experience preferred. • Two or more years long-term care experience preferred. • One or more years MDS/RAP experience preferred. • Ability to utilize both local and corporate resources in the execution of job responsibilities. • Ability to develop an adequate knowledge of state, federal and regulatory requirements related to job. • Must be able to complete the IHS MDS competency within first 90 days of employment. SUPERVISION RECEIVED Receives direction from the Director of Nursing. EXPOSURE RISK The Nursing Assessment Coordinator is at high risk for exposure to blood and body fluids when actively interacting with and/or providing care to patients/residents. Performance of administrative duties results in minimal exposure risk. WORKING CONDITIONS * Works inside the facility throughout the nursing service area, including the medication rooms, nurses’ stations and the patient/resident rooms. • Sits, stands, b&ids, lifts, reaches, walks and moves intermittently during the working hours. • Is subject to frequent interruptions. 4.1s subject to a quiet-to-moderate noise level due to phones, mechanical alarms, and occasional construction work. 5.1s involved with patients/residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. 6.1s subject to hostile and emotionally upset patients/residents, family members, personnel, and visitors. * Communicates with the medical staff, nursing personnel, and other department supervisors. • Works beyond normal working hours, and in other positions temporarily, when necessary. 9. Is subject to hazards in the work area including burns from equipment, odors, exposure to sharp instruments, falls, chemical cleansers etc., throughout the working hours. 10. Is subject to exposure to infectious waste, diseases or conditions. * Maintains a liaison with the patients/residents, families, support departments, etc., to adequately plan for the patient's/resident's needs. • May be required to wear a facemask, gown, or gloves. • May be required to perform as Charge/Unit Nurse in absence of scheduled nurse if replacement cannot be found. SPECIFIC REQUIREMENTS * Must possess a current, unencumbered license to practice as an RN in the state of practice. • Must be able tb read, write, speak and understand the English language. • Must possess the ability to make independent decisions when necessary. • Must be able to relate information concerning a patient's/resident's condition. PHYSICAL AND SENSORY REQUIREMENTS (With or Without the Aid of Mechanical Devices) * Must be able to move intermittently throughout the workday. • Must be able to speak the English language and communicate effectively with others. • Must be able to cope with the mental and emotional stress of the position. • Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of this position can be fully met (i.e., accurately read measurements on patient/resident related equipment such as thermometers, monitors, gauges). • Must be able to function independently, have personal integrity, flexibility, and the ability to work effectively with patients/residents, personnel and support agencies. • Must be in good general health and demonstrate emotional stability. • Must be able to relate and work with the disabled, ill, elderly, emotionally upset, and at times hostile people within the facility. • Must be able to lift, push, pull, and move a minimum o5 50 pounds. • Must be able to assist with the evacuation of patients/residents.
