Part-Time RN Care Manager (CCM + RPM) — Hybrid | 10–20 hrs/week | 1099

We’re a small, patient-centered team that values consistent follow-through, strong documentation, and proactive communication. The Role We are hiring a part-time RN Care Manager for 10–20 hours per week in a hybrid position. After training, the role will include approximately 1 clinic day/week in Tuscaloosa, with the remaining work completed remotely from home. This position is ideal for someone who is responsible, organized, self-directed, and comfortable working independently in a healthcare coordination environment. Schedule Expectations • Clinic day (Tuscaloosa): • At least 8:30 AM–12:00 PM (Mon–Th) or 8:30 AM–5:00 PM (Mon–Th) on clinic days • Remote schedule: Flexible, but ideally able to work 1–2 hours daily • Weekly hours target: 10–20 hours/week Key Responsibilities • Conduct CCM and RPM patient outreach and follow-up calls • Support patient enrollment and onboarding into care management programs • Document calls and care plan updates thoroughly and on time • Track tasks, documentation, and follow-ups consistently • Communicate clearly with the supervising clinician (you’ll report directly to the owner) • Work efficiently and independently in an autonomous setting • Maintain organized reporting and workflows using Excel and shared documents Qualifications • Active Alabama RN license (required) • Strong communication skills and reliable responsiveness • Comfortable working independently with minimal supervision • Strong documentation habits and attention to detail • Proficiency with Excel and either Google Workspace and/or Microsoft Office • Experience in CCM, RPM, primary care, care coordination, or case management is a plus (not required) What We’re Looking For This role requires someone who can: ✅ manage time independently ✅ follow through consistently ✅ document accurately ✅ communicate proactively How to Apply Please apply with your resume and include: • Your weekly availability • Whether you can commit to 1 clinic day/week after training • Any prior care coordination / CCM / RPM experience Apply tot his job
