Pharmacist – Medication Therapy Management, MTM
Job Description: • Perform telephonic comprehensive medication reviews, which includes but is not limited to ensuring a complete medication reconciliation is completed while performing clinical assessments on applicable conditions for each targeted member • Evaluate the medical history, current health status, and medication-related needs of patients to develop and provide personalized medication therapy plans • Identify, document, and facilitate resolution of identified medication related problems • Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy • Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems • Build and maintain positive relationships with internal and external customers including physicians and nurses • Other responsibilities as directed by supervisor and/or manager Requirements: • Must be located in either AZ, HI, TX, IL, FL, or PA and have an active pharmacist license in that state • 1+ years in Retail, LTC, Hospital or PBM pharmacy experience, or other pharmacist-to-patient consultation experience • Experience working with patients with chronic disease states, especially diabetes • Experience working as a pharmacist in a call center or other remote environment, with proven ability to meet goals in a remote environment • Adequate home internet required • High Speed Internet preferred • Previous pharmacy experience delivering MTM, specifically using Outcomes MTM Connect platform • Advanced problem solving and decision making skills • Advanced knowledge of medical terminology • Advanced collaboration and teamwork skills • Advanced growth mindset skills (agility and developing yourself and others) • Advanced skills in execution and delivery (planning, delivering, and supporting) • Ability to consider the relative costs and benefits of potential actions to choose the most appropriate option • Advanced digital literacy skills Benefits: • Affordable medical plan options • 401(k) plan (including matching company contributions) • Employee stock purchase plan • No-cost programs for all colleagues including wellness screenings, tobacco cessation and weight management programs, confidential counseling and financial coaching • Paid time off • Flexible work schedules • Family leave • Dependent care resources • Colleague assistance programs • Tuition assistance • Retiree medical access Apply tot his job
