Job Title: Pharmacy Consultant Facility: Sky’s Pediatric Day Center (PDHC) Location: Merced, California Position Type: Consultant / Contract (Non-employee) Patient Census: Up to 6 pediatric patients Position Overview Sky’s Pediatric Day Center (Sky’s Pediatrics) is a licensed Pediatric Day Health Center (PDHC) in Merced, California, serving medically fragile children under the care of a primary care provider. We are seeking a qualified Pharmacy Consultant to collaborate with our interdisciplinary team in supporting safe, effective, and compliant medication management for our pediatric population. The Pharmacy Consultant will serve in an advisory role, assisting with medication-related clinical planning and participating in patient care discussions as part of our Patient Care Committee. Key Responsibilities The Pharmacy Consultant will be responsible for the following duties: • Medication Guidance and Consultation • Provide professional guidance and recommendations related to medication orders, medication regimens, and medication management for pediatric patients. • Support clinical staff in understanding medication indications, dosing considerations, potential interactions, and administration concerns appropriate for the PDHC setting. • Availability for Consultation • Be available by phone Monday through Friday during standard business hours for medication-related questions, guidance, and consultation as needed. • Patient Care Committee Participation • Participate in quarterly Patient Care Committee meetings to review and discuss patient care plans, medication considerations, and overall clinical coordination for patients served at Sky’s Pediatrics. • Collaborative Care Planning • Collaborate with nursing leadership, primary care providers, and other members of the interdisciplinary team to support individualized patient care planning. • Regulatory and Best Practice Support • Provide guidance consistent with applicable state and federal regulations, professional standards, and best practices related to pediatric medication management. Qualifications • Current, active Pharmacist license in the State of California (in good standing). • Experience or demonstrated competence in pediatric pharmacotherapy preferred. • Knowledge of medication management principles in outpatient, pediatric, or healthcare facility settings. • Ability to collaborate effectively with interdisciplinary healthcare teams. • Strong communication skills, both verbal and written. • Ability to provide consultation services in a timely and professional manner. Additional Information • Sky’s Pediatric Day Center currently serves a small patient census (no more than 6 patients), all of whom are under the care of a primary care provider. • This role is advisory and consultative in nature and does not involve direct dispensing of medications.
