Proactive Titration Medical Assistant - Weekend hours

The Cadence Health team seeks a Proactive Titration Clinical Navigator, as part of our expanding team, to play a critical role in enhancing the patient experience and ensuring the smooth delivery of care. The Proactive Titration Clinical Navigator will be responsible for contacting patients to discuss disease-specific medication recommendations in collaboration with Cadence NPs and the patient's physician. This role will be required to work Saturday - Tuesday 8:00AM - 7:00PM EST OR Saturday - Wednesday 8:00AM - 5:00PM EST. WHAT YOU’LL DO: • Reach out to patients to review medication titration recommendations provided by Cadence Nurse Practitioners, ensuring clear communication and adherence to treatment plans. • Every healthcare interaction with Cadence must be an exceptional experience that prioritizes patient well-being and aligns with our health system partners' goals. • Take ownership of your tasks while ensuring high standards of accuracy and professionalism. • Maintain precise, detailed documentation of patient interactions in compliance with HIPAA and Cadence's clinical protocols. • Use your expertise to guide patients in understanding disease-specific medications and how they relate to their care plans. • Show up on time and consistently meet deliverables, ensuring patient care and service are never compromised. • Provide timely, clear, and empathetic communication with patients and healthcare providers. • Manage your responsibilities independently while maintaining high performance and efficiency. WHAT YOU’LL NEED: • Active Certification as a Medical Assistant from an accredited association. (AAMA, AMT, NHA or NCCT). • 5+ years of experience as a Medical Assistant. • Experience maintaining HIPAA-compliant environments and protecting patient confidentiality. • Must have a HIPAA-compliant home office setup with reliable internet, including stable Wi-Fi and strong upload/download speeds. • Highly skilled in written, verbal, and interpersonal communication, with the ability to engage patients and healthcare providers effectively. • Strong track record of working independently, managing time effectively, and fulfilling job duties with minimal supervision. • Consistent attendance and adherence to work schedules to ensure timely and effective patient care.
