We partner with health systems to deliver CMS-aligned Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services that improve outcomes and reduce clinician burden. About the Role The RPM Medical Assistant supports patients enrolled in chronic care programs by helping them onboard monitoring devices, engage with the CIPRA.ai app, understand expectations, and stay adherent to treatment plans. This is a highly patient-facing role requiring excellent communication skills, comfort with technology, and the ability to identify when clinical escalation is needed. This is a part-time role designed for schedule flexibility while creating meaningful impact for patients managing chronic illness. Key Responsibilities Patient Enrollment & Outreach • Receive provider referrals and conduct patient enrollment (consent, eligibility checks, program overview). • Communicate program expectations—including device use, frequency of blood pressure / glucose readings, and engagement targets. • Support bilingual (English/Spanish) patient communication when needed. RPM Device & App Onboarding • Guide patients through setup and troubleshooting of connected devices (e.g., Omron BP monitors, Dexcom/Libre CGM readers, Bluetooth glucometers). • Educate patients on the CIPRA.ai app and data transmission requirements. • Ensure successful data flow and escalate technical issues as appropriate. Monitoring, Follow-Up & RPM Support • Review RPM dashboards, vital sign trends, and alerts; escalate concerns per protocol. • Conduct follow-up calls or messages to support adherence to: • Care plans and lifestyle recommendations • Medication routines • Required glucose/BP readings • Encourage patient engagement with app-generated guidance and coaching. Documentation & CMS Compliance • Log patient interactions, outreach attempts, education provided, and care time. • Document outcomes to support RPM/CCM billing requirements. • Maintain accurate records following HIPAA and organizational policies. Qualifications • Certified Medical Assistant (CMA, RMA, or equivalent) with clinical support experience in primary care, chronic care, or patient engagement. • Fluency in English and Spanish (verbal and written). • 1+ years of experience in patient support, chronic disease management, care coordination, or device onboarding. • Comfortable using digital tools, mobile apps, and electronic health platforms. • Empathetic communicator able to motivate patients and reinforce behavioral change. • Experience supporting hypertension or diabetes populations is a plus.
