Scheduling & Logistics Coordinator

Job Description: • Manage and schedule patient visits in Skilled Nursing Facilities (SNFs), ensuring optimal and timely provider coverage. • Oversee appointment confirmations, cancellations, and rescheduling, maintaining updated records at all times. • Communicate directly with patients to obtain appointment consent when needed. • Verify SNF patient eligibility and insurance coverage prior to scheduling appointments. • Maintain complete and accurate records of eligibility status within internal databases. • Collaborate closely with billing and intake teams to correct discrepancies or incomplete information. • Ensure adherence to payer requirements and company eligibility standards. • Maintain detailed documentation of schedules, verification processes, and communication logs. • Generate and analyze scheduling and operational reports to identify trends and areas for improvement. • Support data migration and reconciliation across multiple platforms. • Contribute to continuous process improvement initiatives within the scheduling and eligibility functions. Requirements: • Proficiency in scheduling tools and Electronic Health Record (EHR) systems. • Exceptional attention to detail and ability to manage competing priorities. • Strong communication, interpersonal, and problem-solving skills. • Customer-oriented approach with the ability to handle sensitive patient information professionally. • Knowledge of healthcare eligibility verification and insurance processes is a plus. • Experience in scheduling or logistics coordination within a healthcare or SNF environment preferred. • Associate degree in Healthcare Administration, Business, or related field preferred. • Minimum of 2 years of experience in scheduling, eligibility verification, or healthcare operations—preferably in SNFs. • Comfortable working collaboratively across departments in a fast-paced environment.
