This position requires a candidate who resides in the Southwest Side of Chicago, IL and has a deep understanding of the local community. Conduct analysis of case management processes and procedures to identify areas for improvement and implement solutions. Collaborate with team members to develop and implement strategies for efficient and effective healthcare delivery. Utilize data and analytics to track and monitor case management performance and identify opportunities for optimization. Build and maintain relationships with local community resources to support the case management process. Serve as a subject matter expert on local healthcare resources and services in the Southwest Side of Chicago. Provide support and guidance to case managers to ensure compliance with company policies and procedures. Develop and maintain reports and dashboards to track key performance indicators and present findings to management. Conduct regular audits to ensure accuracy and completeness of case management documentation. Participate in training and development programs to stay up-to-date on industry trends and best practices.
