Serve as a primary point of contact for members, providing personalized support and guidance throughout their healthcare journey. Utilize strong knowledge of healthcare systems and resources to help members access the care and services they need. Conduct thorough assessments of members' needs and develop individualized care plans. Collaborate with healthcare providers and community resources to coordinate and facilitate services. Advocate for members to ensure they receive timely and appropriate care. Monitor and track members' progress and make necessary adjustments to care plans. Communicate effectively with members, their families, and healthcare professionals to provide updates and address any concerns. Maintain accurate and detailed records of member interactions and interventions. Stay current on healthcare policies, regulations, and resources to provide up-to-date support and guidance to members. Participate in team meetings and training to continuously improve skills and knowledge. Demonstrate empathy, compassion, and professionalism in all interactions with members. Adhere to all ethical standards and maintain confidentiality of member information. Collaborate with other departments and teams within Aetna to ensure seamless and efficient care for members. Identify and report any potential issues or concerns related to member care or service delivery.
