In this role, you will serve as a CHW Justice System and Court Liaison and will be the primary point of contact to respond to inquiries from the Health Care Authority (HCA), Child, Youth Families Department (CYFD), Members/families, Caregivers, and Providers, and will work collaboratively to address concerns related to the delivery of and access to services for Children in State Custody (CISC) Members and Child Members in out-of-home placements. Responsibilities • Identify UHC justice members prior to release to provide resources and support for their reintegration • Communicate with all justice stakeholders the required information to ensure quality coordinated care and services are provided expeditiously to all members • Provide education and support to members and their families, who are justice or court involved, helping navigate the health care system for physical and behavioral health services • Facilitate referrals to the justice care coordinator who develops a care plan and sets goals for member care • Act as a Community Liaison to the Probation, Court system, Detention and Corrections departments • Complete video visitation and assessment with members who are incarcerated to prepare members for reentry into the community • Document all member assessments and referrals provided • Participate in Interdisciplinary team meetings and discharge planning rounds with justice facilities and provide information to assist with safe transitions of care • Maintain linkages with community partners and other county resources in relation to education, housing, health care, recreation, and socialization • Partner with care team (community, providers, internal staff) • Conduct Health Risk Assessments (HRA) • Perform other duties as assigned Requirements • High School Diploma/GED (or higher) OR 3+ years of Criminal Justice/Social Work/public health experience • 1+ years of experience working with justice systems and/or involved members and their families • 1+ years of experience writing reports using both quantitative and qualitative data analysis • 1+ years of experience preparing and presenting educational materials • 1+ years of knowledge and understanding with health equity • Intermediate level of proficiency with Microsoft Office (PowerPoint, Word, & Excel) • Ability to attend meetings, community activities/events across New Mexico; sometimes before or after standard work hours and on weekends • Possess a valid New Mexico Driver's license and current automobile insurance with access to reliable transportation • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI) • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection Nice-to-haves • Community Health Worker (CHW) Accreditation • Experience working in Managed Care • Experience working in team-based care • Knowledge of Medicaid population • Bilingual Spanish and English Benefits • Paid Time Off which you start to accrue with your first pay period plus 8 Paid Holidays • Medical Plan options along with participation in a Health Spending Account or a Health Saving account • Dental, Vision, Life & AD&D Insurance along with Short-term disability and Long-Term Disability coverage • 401(k) Savings Plan, Employee Stock Purchase Plan • Education Reimbursement • Employee Discounts • Employee Assistance Program • Employee Referral Bonus Program • Voluntary Benefits (pet insurance, legal insurance, LTC Insurance, etc.) Apply tot his job
