Clinical Case Manager, RN Field - Bilingual

Provide personalized and comprehensive clinical case management services to members. Utilize clinical expertise to assess and coordinate care for members. Collaborate with interdisciplinary team to ensure quality and timely care for members. Maintain accurate and timely documentation of member care and progress. Conduct in-person visits with members to assess needs and develop care plans. Communicate with members, caregivers, and healthcare providers to coordinate care and address any concerns. Educate members on their conditions, treatment plans, and available resources. Identify and address any barriers to care and develop strategies to overcome them. Advocate for members to ensure they receive appropriate and necessary care. Stay up-to-date on healthcare regulations and guidelines to ensure compliance. Maintain confidentiality and adhere to HIPAA regulations. Bilingual fluency in English and Spanish to effectively communicate with members. Display compassion and empathy towards members and their families. Maintain a positive and professional attitude while working with members and colleagues. Participate in ongoing training and development to improve skills and knowledge. Adhere to company policies and procedures. Flexibility to work in a field-based role and travel to meet with members.
