Key Responsibilities Conduct thorough assessments of individuals' needs and qualifications using care management tools and data/information audit Identify high-risk factors and service needs that may impact individual outcomes and care planning, referring to clinical case management or crisis intervention as necessary Facilitate and implement assigned care plan activities, monitoring progress and adjusting plans as needed Collaborate with caseworkers, supervisors, Clinical Directors, and other health programs to overcome barriers and achieve objectives Present cases at case conferences to obtain multidisciplinary review and achieve optimal results Identify and escalate quality of care issues through established channels Utilize discussion skills to secure appropriate choices and services necessary to meet individuals' benefits and healthcare needs Employ influencing/ motivational interviewing skills to ensure maximum individual engagement and promote lifestyle/behavior changes to achieve optimal health Provide education, information, and support to empower individuals to make ongoing independent clinical or healthy lifestyle decisions Assist individuals in actively and knowledgeably participating with their healthcare provider in healthcare direction Essential Qualifications 2+ years of experience in behavioral health or social services 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook) Bachelor's degree or non-licensed master's level clinician with a degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) or equivalent experience Preferred Qualifications Case management and discharge planning experience Managed Care experience Career Growth Opportunities and Learning Benefits At blithequark, we are committed to the growth and development of our employees.
