Job Overview We are seeking an experienced Field-Based Home and Community-Based Services Clinical Coordinator Registered Nurse to join our team in Denton County, Texas. As a Field-Based Clinical Coordinator, you will have the flexibility to work remotely, with approximately 75% travel in the field, and a home-based office. You will be responsible for assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
