Key Responsibilities Deliver Coordinated, Patient-Focused Virtual Care Management: Utilize phone or video consultations to enhance members' health outcomes, providing personalized care and support. Collaborate Effectively with the Care Team: Work closely with members and the multidisciplinary care team to formulate impactful care plans that support the achievement of health objectives. Guide Members through Complex Medical Conditions: Educate members on treatment options, benefits, and the healthcare system, empowering them to make informed decisions about their care. Establish Partnerships with Local Providers: Foster relationships with local providers to ensure seamless care coordination and comprehensive support for our members. Provide Compassionate Follow-Up Care: Nurture supportive relationships with members, offering ongoing follow-up care and emotional support during critical healthcare events. Assist Members during Critical Healthcare Events: Coordinate case management and provide essential resources to address members' clinical and social needs during hospitalizations and rehabilitation. Integrate Essential Resources: Comprehensively address members' clinical and social needs by integrating essential resources and services. Required Skills and Qualifications To succeed in this role, you will need to possess the following skills and qualifications: Bachelor of Science in Nursing (BSN): A degree in nursing is essential for this role, and a BSN is preferred. Current CCM Certification: Mandatory certification as a Certified Case Manager (CCM) is required for this position. Strong Communication and Advocacy Capabilities: Excellent communication and advocacy skills are essential for effective collaboration with members and the care team. Proficiency in Managing Complex Medical Situations: Ability to navigate complex medical situations and healthcare systems is crucial for success in this role.
