Job Overview We are seeking experienced and caring Personal Care Assistants (PCAs) and Home Health Aides (HHAs) to join our team of dedicated professionals in Jamestown, NY. Key Responsibilities Provide personalized care and support to clients, including assistance with bathing, oral hygiene, preparation and feeding of meals, dressing and undressing, ambulation, and other activities as needed Operate mechanical equipment such as Hoyer lifts, hospital beds, wheelchairs, and other assistive devices safely and effectively Record and report changes in the client's physical condition, behavior, or appearance to supervisors or case coordinators in a timely and accurate manner Problem-solve and create solutions to promote healthy lifestyle habits and provide general companionship and advice to clients Follow and record care plan activities and visit documentation in accordance with company and Medicaid policies Assist clients with medication management, as needed, and ensure that all medications are taken as prescribed Provide emotional support and companionship to clients, helping to reduce feelings of loneliness and isolation Collaborate with other healthcare professionals, such as nurses and therapists, to ensure comprehensive care and support for clients Essential Qualifications Valid PCA/HHA certifications and required medical documentation Pass background check and approved NYS Home Care Registry status Excellent judgment and demonstrated discretion handling confidential information Valid photo ID Ability to work independently and as part of a team Strong communication and interpersonal skills Compassion, empathy, and a commitment to delivering exceptional care Preferred Qualifications Previous experience working as a PCA or HHA in a home care setting Knowledge of Medicaid policies and procedures Experience working with individuals with disabilities or chronic illnesses Ability to speak multiple languages (optional) CPR/First Aid certification (optional) Skills and Competencies Strong observational and reporting skills Ability to problem-solve and think critically Excellent time management and organizational skills Ability to work in a fast-paced environment and prioritize tasks effectively Strong attention to detail and ability to maintain accurate records Ability to lift, transfer, and reposition clients, as needed Career Growth Opportunities and Learning Benefits At Crane Home Care, we are committed to supporting the professional growth and development of our staff.
