Job Summary We're currently seeking an experienced Registered Nurse - Case Management to join our team on a travel assignment. As a key member of our case management team, you'll play a vital role in coordinating patient care across multiple settings and services. Key Responsibilities Patient Care Coordination: Collaborate with healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient. Care Plan Development: Conduct thorough assessments of patient needs and create tailored care plans that prioritize patient-centered care. Interdisciplinary Collaboration: Work closely with healthcare professionals, including physicians, therapists, and other nurses, to ensure comprehensive patient care. Patient Progress Monitoring: Continuously monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Patient and Family Education: Provide patients and their families with education and resources to support their care plans and promote healthy behaviors. Documentation and Reporting: Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational policies. Requirements and Qualifications Essential Qualifications: RN Licensure: Current RN licensure in the state of Oklahoma or eligible for licensure. Experience: At least 2 years of experience in case management or a related field. BLS Certification: Basic Life Support (BLS) certification. National and State RN Certifications: National and state RN certifications as required. Preferred Qualifications: Case Management Certification: Certification in case management, such as CCM or ACM. Experience with Electronic Health Records: Proficiency with electronic health records (EHRs) and other healthcare software. Skills and Competencies To succeed in this role, you'll need: Excellent Communication Skills: Strong verbal and written communication skills, with the ability to effectively collaborate with healthcare professionals, patients, and families. Analytical and Problem-Solving Skills: Ability to analyze complex patient data, identify trends, and develop creative solutions to drive positive outcomes.
