Job Summary We're seeking a highly skilled and compassionate **Registered Nurse Case Manager - Complex Care** to join our team in Washington State. As a remote-based position with occasional field visits, this role offers the flexibility to work from home while still engaging with our community. Key Responsibilities Assess and Interpret Customer Needs: Evaluate customer requirements and develop comprehensive plans to address their complex needs. Requirements and Qualifications Essential Qualifications: Active, Unrestricted RN License: Current, unrestricted Registered Nurse license in the state of Washington. Microsoft Office Proficiency: Proficiency in Microsoft Word and Outlook. Field Visit Requirements: Ability to travel up to 25% within a 60-mile radius from residence to make field visits. Residency Requirement: Currently live in Tacoma, Olympia, Seattle, or Everett, WA. Preferred Qualifications: Certified Case Manager (CCM) Certification: Certification as a Case Manager is highly desirable. Community Health Experience: 1+ year of experience working in a community health setting or providing community-based care management. Complex Care Experience: Experience managing populations with complex medical or behavioral needs. Home Care Based Services Experience: Experience working with Home Care Based Services and/or patients in community and home-based settings. Case Management Experience: Experience with case management, discharge planning, and related activities. Field-Based Work Experience: Experience with field-based work and ability to work independently. Familiarity with NCQA Standards: Knowledge of NCQA standards and guidelines.
