Job Summary As a Virtual RN, you'll play a critical role in our innovative patient care model, utilizing your clinical expertise, critical thinking skills, and knowledge of care coordination and adult education to deliver seamless, effective, and efficient care to our patients. You'll work closely with our healthcare team to assess, plan, implement, coordinate, monitor, and evaluate patient care plans from admission to discharge, ensuring that our patients receive the highest quality care possible. Key Responsibilities Utilize clinical knowledge and critical thinking skills to coordinate and implement patient care plans, ensuring seamless transitions and high-quality care. Assess patient needs and develop individualized care plans in collaboration with the healthcare team. Provide education and support to patients and families, promoting health literacy and self-care. Monitor patient progress and adjust care plans as needed to ensure optimal outcomes. Collaborate with healthcare providers to ensure effective communication and coordination of care. Utilize electronic medical records (EMRs) and other healthcare technology to document and manage patient care. Essential Qualifications To be successful in this role, you'll need: A current Registered Nurse license in the State of Idaho, as defined by the Idaho State Board of Nursing. A minimum of eight (8) years of experience in health condition management, with a focus on adult condition management. Experience with electronic medical records (EMRs) and other healthcare technology. A graduate degree from an accredited school of nursing (Bachelor of Science in Nursing preferred). Preferred Qualifications While not required, the following qualifications are highly desirable: A Bachelor of Science in Nursing degree. Experience working in a virtual or telehealth environment. Knowledge of care coordination principles and adult education. Skills and Competencies To excel as a Virtual RN, you'll need: Strong clinical knowledge and critical thinking skills. Excellent communication and interpersonal skills. The ability to work effectively in a fast-paced, dynamic environment. Proficiency in EMR systems and other healthcare technology.
