Job Overview As a Social Determinants of Health Community Strategist, you will be responsible for providing strategic leadership and direction to identify, develop, and sustain community partnerships that incorporate social determinants of health. Your expertise will be instrumental in establishing and maintaining relationships with key external stakeholders, including hospitals, government agencies, and community-based organizations. Key Responsibilities Develop and execute integrated and effective strategies and programs to improve health outcomes and reduce health disparities. Establish and maintain relationships with key external stakeholders, including hospitals, government agencies, and community-based organizations. Identify and develop community partnerships that incorporate social determinants of health to drive meaningful change. Provide leadership and strategic direction to sustain community partnerships and ensure their continued impact. Develop and implement programs that address social determinants of health, such as housing, education, and employment. Collaborate with a wide range of individuals and teams to leverage collective expertise and drive collaborative solutions. Analyze data to evaluate the impact of social determinants of health initiatives and inform future strategies. Develop and share best practices for addressing social determinants of health, promoting knowledge sharing and innovation. Monitor health trends to identify opportunities to improve health outcomes and reduce health disparities. Prepare reports and presentations to stakeholders, communicating the value and impact of our community partnerships. Essential Qualifications To be successful in this role, you should possess: A strong background in public health and/or social determinants of health, with a deep understanding of the complex factors that influence health outcomes. Excellent interpersonal, communication, and organizational skills, with the ability to collaborate with diverse stakeholders and teams. A proven track record of developing and executing effective strategies and programs that drive meaningful change. Experience in establishing and maintaining relationships with key external stakeholders, including hospitals, government agencies, and community-based organizations. Strong analytical skills, with the ability to analyze data and evaluate the impact of social determinants of health initiatives. Preferred Qualifications While not essential, the following qualifications will be considered a plus: A graduate degree in public health, social work, or a related field. Experience working in the healthcare industry, particularly in a role focused on community partnerships and social determinants of health. Knowledge of healthcare trends and policies, with a deep understanding of the complex factors that influence health outcomes.
