Job Description

The Remote Nurse Health Educator will work closely with patients to provide education on managing chronic conditions, understanding medications, and promoting overall healthy lifestyles. In this role, you will use your clinical knowledge and communication skills to deliver high-quality care and education remotely.

You will be responsible for creating and conducting educational programs, developing patient care plans, and offering support to patients and their families. Your goal will be to empower patients to take control of their health and improve their quality of life.

Requirements

– Active Registered Nurse (RN) license in the state of residence.

– Minimum of 3 years of nursing experience.

– Strong knowledge of chronic disease management.

– Excellent communication and interpersonal skills.

– Ability to work independently and manage time effectively.

– Proficiency with telehealth technologies and electronic health records (EHR).

– Commitment to patient-centered care and continuous improvement.

Responsibilities

– Provide health education to patients and their families via phone, video calls, and online resources.

– Develop and implement personalized care plans and educational materials.

– Conduct regular follow-ups to monitor patient progress and adjust care plans as necessary.

– Collaborate with healthcare providers and other team members to ensure comprehensive care.

– Stay current with the latest healthcare guidelines and best practices.

– Document all patient interactions and educational sessions accurately in the EHR.

– Participate in ongoing training and professional development opportunities.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) required.

– Master? Degree in Nursing or Healthcare Education preferred.

Experience

– At least 3 years of clinical nursing experience.

– Experience in health education or patient teaching preferred.
