Job Description
CVS Health is seeking a dedicated and compassionate Telephonic Nurse Coordinator to join our dynamic team. As a Telephonic Nurse Coordinator, you will play a crucial role in providing high-quality care and support to our patients over the phone. This position is ideal for an experienced nurse who is looking to make a positive impact on patient health while working remotely. You will be responsible for managing patient cases, providing medical advice, and coordinating care plans to ensure the best possible outcomes for our patients.
Key Responsibilities:
- Conduct telephonic assessments of patients’ health status and needs.
- Provide education and counseling to patients regarding their medical conditions and treatment plans.
- Coordinate care with other healthcare providers and services to ensure comprehensive patient care.
- Monitor and follow up on patients’ progress and adherence to treatment plans.
- Document patient interactions and care plans accurately in the electronic medical record system.
- Respond to patient inquiries and concerns in a timely and professional manner.
- Participate in continuous education and training to stay current with best practices and medical guidelines.
Requirements:
- Active and unrestricted Registered Nurse (RN) license.
- A minimum of 3 years of clinical nursing experience, preferably in a telephonic or case management role.
- Strong communication and interpersonal skills.
- Ability to work independently and manage multiple tasks efficiently.
- Proficiency with electronic medical record systems and basic computer skills.
- Excellent organizational and time-management skills.
- Commitment to providing high-quality patient care and support.
Educational Qualifications:
- Bachelor of Science in Nursing (BSN) preferred.
- Additional certifications in case management or related fields are a plus.
Experience:
- Minimum 3 years of clinical nursing experience.
- Experience in telephonic nursing or case management preferred.
