Key Responsibilities As a Home Health Physical Therapist, your primary responsibilities will include: Planning and administering prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function. Testing/screening patients' physical strengths to assist physicians in evaluating patients' levels of function and recording findings to develop or pursue treatment programs and establish measurable training objectives. Developing/implementing a conditioning/rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team. Adjusting treatment as needed to achieve maximum results. Confering with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment. Providing Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans that reflect the current plan of care as related to therapy, supervising/evaluating staff performance. Monitoring the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers as part of supervisory/leadership responsibilities. Accurately, promptly, and thoroughly documenting patients' care observations, interventions, and evaluations. Assuring that interim (verbal) orders received from physicians and physical therapists are promptly and accurately documented, submitted for physician signature, and implemented. Reporting patients' progress to patients' physicians, Clinical Managers, staff, patients, and families. Submitting evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff. Requirements and Qualifications To be successful in this role, you will need: Essential Qualifications: A degree from an accredited Physical Therapy Program (approved by the APTA). Current and unrestricted Physical Therapy licensure. Current CPR certification. Strong organizational and communication skills. A valid driver's license, auto insurance, and reliable transportation. Preferred Qualifications: Minimum of one year of physical therapy experience. Home Health experience.
