As a Population Health Strategic Consultant - NCQA Lead, you'll be at the forefront of these efforts, working to develop and implement strategic initiatives that drive positive change. Key Responsibilities Assess and interpret customer needs and requirements, identifying solutions to non-standard requests and problems Solve moderately complex problems and conduct analyses, working with minimal guidance and seeking guidance only on the most complex tasks Translate concepts into practice, providing explanations and information to others on difficult issues, and mentor, provide feedback, and guide others Visit providers' offices to educate them on clinical measures, disease prevention programs, and communication resources Provide providers with lists of gaps in member services/screenings to improve the overall health of members Perform chart reviews for HEDIS and State Required Audits Serve as a subject matter expert for assigned HEDIS/CMS measures, assisting with rate improvement and provider knowledge Collaborate with providers to execute the implementation of clinical quality data initiatives related to HEDIS Provide analytical interpretation of HEDIS reporting, including executive summaries to plan and provider groups Assist with performance improvement projects and special projects as assigned Work with patients and their care teams to coordinate change readiness, needs assessment, and develop individualized treatment care plans Implement clinical and quality initiatives, promoting affordability and cost analysis, utilization, and quality data improvements Additional Responsibilities Manage ongoing provider practice and community education on state-specific quality measures Coordinate an interdisciplinary approach to increased provider performance and disease management with the Quality Director Provide education to providers regarding the quality improvement program, analyzing and reviewing quality outcomes at the provider level, and monitoring, measuring, and reporting on key metrics Focus on tasks that occur in accordance with State, CMS, or other requirements as applicable Assist contracted providers with analyzing member care, trending quality compliance, and developing action plans and programs to support provider practices in continuous quality improvement Assist in the development and implementation of disease-specific population health programs Requirements and Qualifications Essential Qualifications Undergraduate degree Current unrestricted RN licensure in the state of Louisiana 3+ years of clinical nursing experience, preferably in a managed care environment Louisiana driver's license with a clean driving record Experience presenting to others Access to high-speed internet Preferred Qualifications CPHQ Certification Proficient with working within a Windows environment, utilizing Outlook, and advanced skills with Microsoft Excel Ability to create spreadsheets in Excel Experience working with provider offices Experience working with the Medicaid/Medicare population Comfort with reading reports and analyzing data What We Offer As a valued member of our team, you'll enjoy a comprehensive benefits package, including: Competitive salary range: $71,600 to $140,600 annually Incentive and recognition programs Equity stock purchase 401k contribution Comprehensive benefits package Our Culture and Values At UnitedHealth Group, we're committed to creating a diverse and inclusive work environment that values the unique perspectives and experiences of all our employees.
