Job Summary As a Home Health RN with Interim HealthCare, you will have the opportunity to work in a fully remote role, with the flexibility to create your own schedule and enjoy a healthy work-life balance. You will provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility. Your responsibilities will include: Conducting patient assessments, documenting progress, and reporting changes to physicians Developing and implementing patient care plans, including education on prescribed medication, therapy, diet, and exercise Coaching family members on patient care, medication, and home safety Coordinating discharge from services and ensuring goals are met Working with a team of physicians and overseeing LPNs, CNAs, and aides providing patient care Key Responsibilities As a Home Health RN with Interim HealthCare, your key responsibilities will include: Providing high-quality, patient-centered care in a home setting Conducting thorough patient assessments and developing comprehensive care plans Collaborating with physicians and other healthcare professionals to ensure seamless care coordination Educating patients and their families on self-care techniques, medication management, and disease prevention Maintaining accurate and up-to-date patient records and documentation Requirements and Qualifications To be successful as a Home Health RN with Interim HealthCare, you will need: A graduate degree from an accredited nursing program and an active RN license in South Dakota A minimum of 2 years of nursing experience, ideally in home healthcare CPR certification Knowledge of state and federal home health regulations Clinical proficiency in nursing specialty, strong communication skills, and compassion Preferred Qualifications While not required, the following qualifications are preferred: Experience with electronic medical records (EMRs) and other healthcare software Certification in a nursing specialty, such as pediatrics or gerontology Experience working with diverse patient populations and cultures What We Offer As a Home Health RN with Interim HealthCare, you can expect a range of benefits and rewards, including: A competitive salary of $27-$33 per hour A $500 sign-on bonus 1:1 nurse-to-patient ratios, allowing for personalized care and attention Flexible assignments and autonomy to create your own schedule Online training and growth opportunities, including CEUs Tuition discounts through Rasmussen University PTO, holiday pay, and 401(k) benefits Career Growth and Learning Benefits At Interim HealthCare, we are committed to supporting the professional growth and development of our nurses.
