[Remote] Care Coordinator

As a Care Coordinator, you will support clients in accessing mental health services, ensuring they are connected with the appropriate care providers and maintaining support throughout their care journey. Responsibilities Provide primary support for clients seeking care to manage their mental and behavioral health wellness Utilize a variety of technologies to ensure the client is connected with the appropriate therapist and maintains the required support through an episode of care Utilize a consultative approach to assist with identifying the correct provider for care Manage inbound and outbound calls and other forms of communication while applying standard guidelines to address or route messages to the appropriate care team member for follow-up when necessary Interact with external partners and healthcare systems to ensure access to care Conduct outreach to referred clients and provide personalized onboarding services and assistance with all aspects of getting into care Assist with care coordination tasks such as appointment scheduling and facilitating communication with providers and other care team members And other responsibilities and ad-hoc projects from time to time based on business needs Skills Strong desire to assist clients and provide support to those seeking mental health care Ability to work with people in vulnerable situations Strong communication skills, both written and verbal Motivated and eager to learn, ability to adapt to new technologies, processes, and workflows Strong problem-solving skills and attention to detail Flexibility to work in a fast-paced, dynamic environment Goal-oriented with a strong drive to achieve results Openness to feedback and a commitment to personal and professional development Familiarity with job-relevant AI platforms such as Gemini, ChatGPT, Claude, GitHub Copilot or other industry-standard AI productivity tools Benefits A generous PTO policy, with a minimum of three weeks off per year A holiday schedule that follows standard U.S. holidays Free therapy coverage benefits to ensure employees have access to the care they need (must be enrolled in a qualifying medical plan to participate) Competitive Medical, Dental, and Vision coverage, with plans to meet every need — including HSA (with $1,100 company contribution) and FSA options Employer-paid short-term disability, long-term disability, life & AD&D, plus coverage of the salary difference for up to seven weeks of short-term disability leave (after the required waiting period) Eight weeks of paid Parental Leave; if the parent also qualifies for STD, this benefit is in addition, allowing for 8–16 weeks of paid leave 401(k) retirement plan with 100% match on up to 4% of base salary, immediately vested Join teammates from across the country at our annual company gathering Company shutdown between Christmas and New Year’s Supplemental life insurance, pet insurance, commuter benefits, and more Company Overview SonderMind is a mental healthcare provider that offers treatment for mental health through therapy and psychiatry.
