Join us in shaping the future of health care and making a meaningful impact at Job Description: We’re seeking a full-time experienced Remote Hospital Pharmacist to join our team to work a 7-on-7-off night shift position. The ideal candidate is someone who possesses strong attention to detail, communication skills, and time management. Essential Duties/Responsibilities: • Reviews and monitors all new medication orders (electronic and written) for accuracy in dosing, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities. • Provides pharmaceutical care to all ages of patients from newborns to the elderly. Understands that there are dosage variations between newborn, pediatric, adult and geriatric patients and knows where to find appropriate information. • Effectively communicates with on-site staff to resolve medication-related issues such as dosage adjustments, clarification requests, drug interactions, and duplications of therapy. • Demonstrates ability to document clinical interventions. • Provides information and support to on-site staff regarding the admixture or compounding of medications upon request. • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medication-related errors. • Answers questions and provides information to physicians, nurses, patients and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
