[Remote] LPN Care Coordinator
Note: The job is a remote job and is open to candidates in USA. Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking an LPN Care Coordinator to join our team. The role involves telephonic and videoconference outreach to members post hospital discharge, focusing on Intensive Care Management services, proactive coaching, and collaboration with RNs to ensure successful member outcomes. Responsibilities Responsible for telephonic/videoconference outreach from post hospital discharge, predictive modeling and other data or claim reports to identify members that would benefit from Intensive Care Management (ICM) services Conducts focused proactive coaching and education Functions as a vital team player to facilitate successful member outcomes Works in collaboration with the RNs to outreach to members to confirm written discharge plans, DME, HHC, transportation arrangements, and post discharge medical appointments; and completes medication reconciliation Completes risk screenings/scripts for identification of member needs/opportunities/SMART goals Refers members to RN Care Manager when gaps in care or medical and/or behavioral needs are identified or may manage average/emerging risk members in collaboration and oversight of an RN as appropriate Responds to telephonic inquiries from members Coaches and assists members in achieving successful self-management skills Identifies appropriate written educational mailings to support the member’s learning Enters appropriate documentation in the electronic care management system Telephonic/videoconference member outreach, education and coaching Review of the appropriate data analytics and post hospital discharge reports to identify, assess and stratify the member’s medical needs and appropriateness for participation in the Intensive Care Management Program Skills Two years' post-secondary schooling 1 to 5 years nursing experience Excellent organizational skills Ability to manage multiple priorities concurrently Strong oral and written communication skills Strong computer and keyboard skills Team player Ability to exercise independent and sound judgment in decision making Active Licensed Practical Nurse License Associates degree Vocational nursing Managed Care experience Knowledge of HUSKY Health contract requirements Benefits Medical, dental and vision coverage options Flexible spending and health savings accounts Group term life insurance A 401(k) plan with company-match and immediate vesting Voluntary accidental injury coverage Tuition reimbursement and continuing education opportunities A generous paid-leave bank and company holidays Wellness program Company Overview In 1995, a group of Federally Qualified Health Centers (FQHCs) came together to form Community Health Network of Connecticut, Inc.® (CHNCT), a not-for-profit managed care organization dedicated to improving medical services and health outcomes for Connecticut’s Medicaid-eligible residents.
