Job Overview As an Aftercare Case Manager, you will play a vital role in providing support and services to children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. Your primary focus will be on risk management during the designated aftercare period, which may include crisis intervention, collaboration with public and private agencies, and coordination of community and educational services. You will work closely with families and children to address concerns, provide guidance, and connect them with essential resources to maintain their permanency. Key Responsibilities Provide support and services to children and families in post-permanency placements, focusing on risk management and crisis intervention as needed Develop and maintain accurate and timely documentation and reports, ensuring compliance with organizational and regulatory requirements Collaborate with public and private agencies to provide supportive services and coordinate community and educational resources Meet with children and families in-person, as determined by the aftercare plan, to support reunification and address concerns Advocate for families and children, assisting them in accessing community resources and maintaining their permanency Participate in ongoing training and professional development to enhance skills and knowledge in areas relevant to the role Essential Qualifications To be successful in this role, you will need: A bachelor's degree in human services or a related field A valid driver's license Compassion, empathy, and a strong commitment to the strength of families Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities Ability to work independently and as part of a team, with a flexible and adaptable approach to changing circumstances Preferred Qualifications While not essential, the following qualifications are highly desirable: Previous experience in a similar role, working with children and families in a social services or non-profit setting Knowledge of community resources and services, with established relationships with local agencies and organizations Experience with crisis intervention and risk management, with the ability to respond effectively in high-pressure situations Bilingual or multilingual skills, with the ability to communicate effectively with diverse populations Skills and Competencies To excel in this role, you will need to demonstrate the following skills and competencies: Communication skills : ability to communicate effectively with children, families, and colleagues, both verbally and in writing Interpersonal skills : ability to build strong relationships with diverse populations, including children, families, and community agencies Problem-solving skills : ability to analyze complex situations, identify solutions, and implement effective interventions Time management skills : ability to prioritize tasks, manage multiple responsibilities, and meet deadlines Adaptability : ability to work in a fast-paced environment, with a flexible and adaptable approach to changing circumstances Career Growth Opportunities and Learning Benefits At TFI Family Services, we are committed to the growth and development of our employees.
