We're seeking an experienced Health Coach to play a vital role in supporting Medicare members through Aetna's Healthy Aging Support Program. As a fully remote position, you'll have the flexibility to work from anywhere while making a meaningful impact on the lives of our members. About the Role As a Health Coach at CVS Health, you'll be responsible for providing health coaching, education, and resources to help Medicare members manage their health, particularly those with chronic conditions. Your expertise and compassion will empower members to take control of their health, and you'll work closely with healthcare providers and Aetna Care Management teams to ensure comprehensive support. Key Responsibilities Educate members on various topics, including Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams, to promote healthy behaviors and disease management. Assist members in actively participating with their healthcare providers and Aetna Care Management teams regarding health decisions, ensuring they receive the care and support they need. Document all member interactions into the appropriate member records, maintaining accurate and up-to-date information. Demonstrate empathy and understanding for the situations faced by members and their caregivers, providing personalized support and guidance. Troubleshoot unavailability of resources and secure support to close member gaps, ensuring they receive the necessary care and services. Follow program workflows consistently and follow up with members to review personal action plans, promoting progress and goal achievement. Essential Qualifications To succeed in this role, you'll need: At least 2 years of experience working as a Health Coach, with a proven track record of supporting individuals with chronic conditions. A Health Coach certification, demonstrating your expertise in health coaching principles and practices. Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time, ensuring consistent and reliable coverage. Preferred Qualifications While not required, the following qualifications are highly preferred: Community health or public health experience, providing a deeper understanding of the social determinants of health and community resources. Managed Care/Medicare/Medicaid experience, offering insight into the healthcare system and the needs of Medicare members. Bilingual skills, enabling you to support members with diverse language needs. A Bachelor's degree, demonstrating a strong foundation in a relevant field such as health education, nursing, or a related discipline. Skills and Competencies To excel as a Health Coach at CVS Health, you'll need: Strong communication and interpersonal skills, with the ability to build rapport with members, healthcare providers, and internal stakeholders. Empathy and compassion, with a deep understanding of the challenges faced by members and their caregivers. Analytical and problem-solving skills, with the ability to troubleshoot issues and develop effective solutions. Organizational and time management skills, with the ability to prioritize tasks and manage multiple member interactions.
