Job Overview We are urgently seeking an experienced Registered Nurse Case Manager to join our Home Health team in Saint Croix Falls, Wisconsin. As a key member of our interdisciplinary team, you will be responsible for integrating healthcare for clients from pre-admission to post-discharge, ensuring seamless coordination and sequencing of care. This is a full-time opportunity with a competitive salary range of $34.00 - $51.00 per hour, depending on experience. Key Responsibilities Integrate healthcare for clients from pre-admission to post-discharge, ensuring a smooth transition and optimal patient outcomes Coordinate and sequence client care, promoting early intervention and effective communication among the interdisciplinary team Evaluate outcomes based on established clinical, financial, and utilization data, identifying areas for improvement and implementing evidence-based solutions Utilize independent nursing judgment when integrating healthcare, demonstrating critical thinking and assertiveness in decision-making Develop and implement individualized care plans, incorporating age-specific structured care methodologies and protocols Communicate effectively with patients, families, and healthcare team members, facilitating collaboration and coordination of care Monitor, evaluate, and trend patient responses, utilizing structured care methodologies and making adjustments as needed Document clearly and accurately, maintaining comprehensive patient records and ensuring compliance with regulatory requirements Essential Qualifications Bachelor's degree in nursing preferred, with graduation from a nationally accredited nursing program (CCNE, ACEN, or NLN CNEA) Minimum of one to two years of professional nursing experience in a clinic care setting, with in-depth knowledge of specialty area preferred Currently holds an unencumbered RN license with the State Board of Nursing, with multistate licensure if in a Nurse Licensure Compact (NLC) state Obtains and maintains required department-specific competencies and certifications Valid driver's license may be required, depending on location Preferred Qualifications Experience in home health or case management, with a strong understanding of reimbursement for services provided Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members Strong critical thinking and problem-solving skills, with the ability to adapt to changing situations and priorities Ability to work independently, with minimal supervision, and as part of an interdisciplinary team Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities Career Growth Opportunities and Learning Benefits At The Good Samaritan Society, we are committed to supporting the growth and development of our employees.
