Job Summary As a Field HSS Clinical Coordinator - RN - Level 1, you'll play a critical role in assessing, planning, and implementing care strategies that are individualized to each patient and directed toward the most appropriate, least restrictive level of care. You'll work closely with patients, families, and healthcare teams to ensure that our members receive high-quality, coordinated care. Key Responsibilities Care Planning and Coordination: Assess, plan, and implement care strategies that are individualized to each patient and directed toward the most appropriate, least restrictive level of care. Referral and Resource Management: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services. Communication and Collaboration: Manage the care plan throughout the continuum of care as a single point of contact. Communicate with all stakeholders, including patients, families, and healthcare teams, to ensure that our members receive high-quality, coordinated care. Patient Advocacy: Advocate for patients and families as needed to ensure that their needs and choices are fully represented and supported by the healthcare team. Travel and Field Work: Travel to assigned regions to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities, or providers' offices. Requirements and Qualifications Essential Qualifications: Current RN License: Current unrestricted RN license in the state of Texas. Experience: 2+ years of experience working within a community health setting or in a healthcare role. Technical Skills: Familiarity with Microsoft Office, including Word, Excel, and Outlook. Transportation and Internet: Reliable transportation and high-speed internet at residence. Preferred Qualifications: Long-Term Care Experience: 1+ year of experience with long-term care services and support, Medicaid, or Medicare. Technical Skills: Ability to create, edit, save, and send documents, spreadsheets, and emails. Knowledge of Integrated Settings: Knowledge of the principles of most integrated settings, including federal and state requirements like the federal home and community-based settings regulations. Residence: Reside within a commutable distance of assigned duties.
