Job Highlights: Position: Medical Social Worker - Home Health Per Diem Location: Edinburgh, IN (Remote work available) Compensation: Competitive salary and benefits package Start Date: Immediate openings available Responsibilities: As a Medical Social Worker with CenterWell Home Health, your primary responsibilities will include: Assessing patients' social and emotional needs, and developing a personalized plan of care to address these needs Evaluating patients' relationships with their medical and nursing needs, financial resources, and community resources Providing direction and supervision to Social Worker Assistants as required Collaborating with the interdisciplinary care team to develop and implement patient care plans Participating in discharge planning and identifying community resources to support patients and their families Maintaining accurate and timely documentation, including case conferences, patient/physician contacts, and visit reports Supervising, instructing, and evaluating the performance of Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards Requirements: To be successful in this role, you'll need: Essential Qualifications: Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education Social Worker licensure in the state of practice (required by state law or regulation) A valid driver's license, auto insurance, and reliable transportation Proof of current CPR certification Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice Preferred Qualifications: Experience with discharge planning and community resource development Knowledge of medications and their correct administration Excellent oral and written communication and interpersonal skills Ability to organize tasks, develop action plans, set priorities, and function under stressful situations Ability to be flexible in work hours and travel locally Skills and Competencies: To excel in this role, you'll need: Strong communication and interpersonal skills Ability to work effectively with patients, families, and healthcare teams Knowledge of community resources and ability to connect patients with these resources Strong organizational and time management skills Ability to prioritize tasks and manage multiple responsibilities Career Growth Opportunities and Learning Benefits: At CenterWell Home Health, we're committed to supporting the growth and development of our team members.
