About the Role: RN Health Coach As an RN Health Coach at Sanford Health, you will play a vital role in guiding care across the continuum and supporting patient care transitions. You will serve as a leader, demonstrating independence, assertiveness, critical thinking, and problem-solving skills, while collaborating with the healthcare team to implement improvement strategies that drive optimal patient care. Key Responsibilities: Utilize a population health perspective to guide care and support patient care transitions, ensuring seamless and effective care coordination. Collaborate with the healthcare team to implement improvement strategies, driving optimal care and outcomes for patients. Apply the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care to patients, addressing their unique needs and circumstances. Work with patients, families, and the care team to promote healthy lifestyles through education, goal setting, and motivational interviewing, empowering them to take an active role in their care. Oversee disease registries and promote the use of Sanford-approved Clinical Practice Guidelines and Protocol Orders, ensuring evidence-based care and best practices. Demonstrate expertise in ambulatory quality measures and serve as a resource for the healthcare team, driving quality improvement initiatives and ensuring high-quality patient care. Maintain required department-specific competencies and certifications, staying up-to-date with the latest developments and advancements in the field. Qualifications and Requirements: To be considered for this role, you must meet the following qualifications: Graduation from a nationally accredited nursing program (CCNE, ACEN, or NLN CNEA) is preferred. A Bachelor's Degree in Nursing is preferred, demonstrating a strong foundation in nursing principles and practices. Currently hold an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Ability to obtain and maintain required department-specific competencies and certifications, demonstrating a commitment to ongoing learning and professional development. Preferred Qualifications: Previous experience in a health coaching or care coordination role, demonstrating a strong understanding of population health principles and practices. Strong clinical knowledge and expertise in ambulatory care, with a focus on quality improvement and patient-centered care. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
