As a Clinical Practice Performance Coordinator, you'll play a vital role in supporting our local quality and provider teams, driving improvements in healthcare outcomes, and enhancing the overall patient experience. About the Role This is a full-time remote opportunity, available to candidates residing in New Hampshire or Maine. As a Clinical Practice Performance Coordinator, you'll work closely with our medical centers and physician practices to manage high-volume UnitedHealthcare Medical & Retirement membership. Your primary focus will be on improving health outcomes, optimizing customer satisfaction, and driving productivity. Key Responsibilities Assist in reviewing medical records to identify Star opportunities for medical staff, involving data collection, data entry, quality monitoring, and chart collection activities. Locate medical screening results and documentation to ensure quality measures are followed, and gaps in care are closed. Track appointments and document information accurately in our supported systems, ensuring timely and efficient data management. Partner with leadership teams and practice staff to develop strategies that support the practice and our members, focusing on preventative health screenings and HEDIS gap closures. Interact with UnitedHealthcare members via telephone to assist with scheduling appointments, closing gaps in care, and addressing chart collection activities. Communicate scheduling challenges and trends that may impact quality outcomes, demonstrating proactive behavior in addressing customer needs. Provide ongoing support and education to team members, helping to remove barriers in care and drive productivity. Manage your time effectively to meet productivity goals, working independently in a virtual setting with minimal supervision. Analyze data from multiple system platforms to identify quality opportunities and address them on a member or provider level. Participate in department campaigns to improve overall quality improvements within measure star ratings or contracts. Perform other duties as assigned, demonstrating flexibility and a willingness to adapt to changing priorities. Essential Qualifications High School Diploma or equivalent (GED). At least 1 year of experience in the healthcare industry. Proficiency in Microsoft Tools, including Word, Outlook, and Excel, with the ability to create, edit, and analyze documents and reports. Knowledge of HIPAA compliance requirements and the ability to maintain confidentiality. Flexibility to work any 8-hour shift schedule during normal business hours (8:00 am - 5:00 pm, Monday - Friday), with occasional overtime as needed. Preferred Qualifications 1+ year of experience in a physician, provider, or medical office setting. 2+ years of telephonic customer service experience. EMR and HEDIS knowledge and experience. Familiarity with medical terminology. Prior experience as a Medical Assistant or LPN. Typing speed of at least 45-50 WPM. Telecommuting Requirements Reside in New Hampshire or Maine. Ability to maintain company-sensitive documents securely. Dedicated work area that is separate from living areas and provides information privacy. Access to a UnitedHealth Group-approved high-speed internet connection. Soft Skills Demonstrated ability to empathize with customers, understand their needs, and align with their realities. Effective active listening skills to develop trust and respect with customers. Self-motivation and internal drive to accomplish goals and recognize what needs to be done to achieve them. Ability to turn situations around and go above and beyond to meet customer needs. Good attendance record and adherence to UnitedHealth Group's Telecommuter Policy.
